Sanitas Medical Center is hiring a Full-Time  Registered Nurse (RN) Care Educator for our Nashville - Cool Springs facility in Nashville, TN.

Job Description:

Sanitas Medical Center is looking for a dedicated  Registered Nurse  ( RN) Care Educator to join our Nashville - Cool Springs team. This role involves performing patient health education at our primary care facility, helping patients achieve self-management through our unique care program. The Care Educator (RN) works with a Care Promoter Medical Assistant (MA) under the direction of the Lead Care Program Nurse to plan, coordinate, and holistically monitor patients needs to positively impact their chronic health conditions.

Location: This full-time position is based at our Nashville - Antioch facility in Nashville, TN.

RN Care Educator Responsibilities:

  • Assist with the identification of patients who qualify for the care program.
  • Perform direct and group patient education.
  • Review patient health risk assessments to formulate nursing care plans.
  • Comply with documentation requirements of the Care Management program.
  • Data entry and supervision of patient health information reports.
  • Perform patient follow-up and monitoring according to health risk status.
  • Promote patient self-management of diseases and behavior modification interventions.
  • Educate patients and families about relevant community resources.
  • Share professional knowledge and learning with team members.
  • Maintain a safe and aseptic environment.
  • Collaborate with other healthcare team members regarding patient care.
  • Adhere to department policies and procedures.
  • Participate in staff and department meetings monthly.
  • Communicate performance status of the care program and supervise vaccine and clinical supply orders and inventory.
  • Attend and assist in company health fairs and events.
  • Some local travel required to cover other facilities within the same area.

Required Skills & Qualifications:

  • BS or Graduate from an accredited school of Nursing.
  • IV Certification required.
  • Active State of Tennessee Registered Nurse License required.
  • Proficient in Microsoft Office programs including WORD, EXCEL.
  • Case Management experience preferred.
  • CPR-BLS Required; ACLS/PALS preferred.
  • 2 years care management/patient education experience preferred.

Physical Demands:

  • Regularly required to stand, walk, and use hands to operate tools and equipment.
  • Must be able to exert up to 10 pounds of force regularly, 30 pounds frequently, and 50 pounds occasionally.
  • Required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull.
  • Specific vision abilities include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus.
  • Subject to environmental conditions, protection from weather but not necessarily temperature changes.
  • Subject to noise sufficient to cause the worker to shout to be heard above ambient noise level.
